What Is This Plant and Am I Taking Proper Care of It?

I am a newbe and have this fern(?) growing in my flowerbed. This is my second year at this residence and both years it starts to grow out lush and green then it gets these spots all over the back and starts to turn a darker dryer color then eventually wilts. Is this a fungus or am I not properly caring for it?
